Welcome to the Records Department Dowload Site for the Covered Bridge Special Education District. The goal of this site is to provide our staff with access to all the current and required forms that you need to fullfill your day to day dealings with our Records Department.

Once you download the appropriate documents, you can either print the form, fill it out and fax it back to us at (812) 462-4377 or you can complete the document using Microsoft Word and email it as an attachment to either Corey or Kristen.
